Found an interesting post that suggests that it was the first round that was shot:

"I find it curious that the dust cover and mounting pin is warped so much that it will not swing open far enough to lay against the receiver, where it sits after the bolt assembly cycles the first time. This would seem to indicate that the failure occurred from firing the first round, not the second. It’s possible that the cover was damaged prior to this incident, but the bulged cover is consistent with an internal explosion while closed, and would be unlikely to occur otherwise. It is possible that the gun was being held close enough to an object to keep the cover from swinging fully open, but then that raises the question of where did the first empty go? -Will"

New information (that I have had to find and then have clarified) concerning the ammo

I posted that I had used factory XM193 before I had the invoice from the ammo purchase. That invoice was unclear as to the designation (it had an item number versus a designation). During that training I used Hornady TAP Practice and PRVI Partizan 5.56 The PRVI Partizan, it has been confirmed by the retailers item number on the invoice, is M193. The internet "experts" that are making unsubstantiated claims as to the cause are doing nothing more than guessing. While some of the theories are interesting and some are even couched as theories versus stone cold fact, lets leave the verdict to those who not only have the knowledge, but have the physical evidence (the actual gun and whats left of the case) to support their claims.

By the way, I was using a Magpul Pmag and it didn't come unseated and was not damaged.

The area Federal Cartridge/ATK rep was here last week, and I discussed this incident at length with him. He said he'll wager large that Colt will blame the ammunition and disavow any culpability...

He did tell me something interesting; while there have indeed been several "blow-ups" with Federal XM193, the ones they have investigated were due to using 5.56mm military-spec ammunition (which XM193 is) in .223 chambers; to include one Wyldie chamber. Naturally, that would exclude the Colt, one would think...

Too bad it is so expensive to submit something to H.P. White labs; there, you will get a totally unbiased assessment.

If colt does that, make it well known on arfcom.

Screw Colt.
There are better companies out there with better Customer Service and equal or better quality parts.

Oh, yes. We all know that arf.com is a bedrock of level-headed, experienced warriors.

The point sailed right over your head.

It may well BE the ammunition. There's no way we will know FOR SURE.

In fact, the damage to parallel's carbine, IIRC, is quite similar to some other ARs that were blown by the Georgia Arms Canned Heat "remanufactured" ammunition some years back. That was traced directly to case failures due to brass from M-249 SAWs that had loose headspace.

My point was, this has no other place to go than to devolve into a finger-pointing debacle.... because the interested parties who will be examining the rifle WILL be biased.
