Question about CHP application..

    I have 2 questions:

    1) for the 'previous residences' section, what is the proper way of filling this out? My parents and I have lived in the same house for the past 14-15 years, except for a few months after Katrina while the house was being fixed. during that time, we lived in one apartment for a little over a month, then a second one for about 6 months, then back to the the house we've been living in for 14+ years. How do I list this? Or do I even have to? Would I have to put 'current house' x date to date after Katrina, 1st appt x to x, 2nd appt x to x, 'current house' date right after that to now?
    and

    2) I am currently unemployed due to going back to school...do I leave the employment section blank?

    Thanks for any help you guys can give. I really appreciate it.

    b/c it was a temp housing, I would omit it (as long as you did not change your DL address). Maybe list that you are a full time student under employment.

    ditto. I've kept the same permanent address in walker since I move around a lot in lafayette, just finding cheaper/nicer apartments and such. I simply put my address there and put Student under employment. No problems with it at all.

    These are reasonable answers. If you're worried, call the CHP hotline 225-925-4867. Sgt. Jason Starnes (the guy in charge of the unit) is a good guy to talk with and very helpful.
