YES several forum members teach the class
What is the (total) cost? (class, and permit application)

Class depends on where you go and what they include with it, anywheres from $80 to $140

State Police Fee $125 (add $50 if you havent lived in Louisiana for the past 15 years, its cheapper if you are 65 years of age

What is the shooting portion of the class consist of?

12 rounds at 6 feet
12 rounds at 10 feet
12 rounds at 15 feet, must perform 1 reload and score 100% hits on NRA B27

Is there a caliber requirement for the shooting portion?

NO, but it is advised you shoot it with what you plan to carry
