Form 1 question

I recently got back my form 1 from the sheriff's office and noticed that he had signed his name where i would sign if i were using a trust or llc (box 8). my question is would I have to go back and have him re-sign the form 1 or can I just put a line through his name and send it off. This is my first nfa item so I am trying to make sure everything is good to go before i send it. Thanks.

I always highlight and prefill everything but the signatures.
