    i just took my CCW class today. i'm hoping to send mines off soon as well.

    but is this correct on what's needed....

    1. fill out application
    2. money order for $125
    3. copy of medical records
    4. passport photo and fingerprints (signed by officer)
    5. photo copy of certificate and license
    6. and then have all the paper work notarized?

    just making sure i'm getting it right...any help would be appreciated.

    Number 5. you need to turn in your original cetificate and you keep the copy.

    if you have had any arrests in the past you will need a disposition for each offense.

    and if you answered yes to some of the medical questions you will need something from your doctor.

    also, if you went through a divorce you need some stuff for that.

    Just telling you what I was told by my instructor. He said with computers and the ease of making fake certificates and documents it would be best to turn in the original. He was getting his info from CHP division of State Police Headquaters so it's your choice.

    I was told that it is easier to just drive up to baton rouge to turn in the app. They do the fingerprints and everything there and they do them untill they are satisfied. I've heard they have been rejecting fingerprints, even from Jefferson Parish Sheriffs office.

    no, never been arrested or divorced (never married). and for the medical questions, even if i answered no to them all, they still need a medical record correct?

    Dropped mine off around the 5th of jan. Got it a few days ago.
    They copy the certificate after they look at it and give yours back if you drop it off.

    Read the application. The medical questionnaire that is completed by your doctor is only required if you answered yes to any of the questions 13-19. It is written in bold directly next to the words "MEDICAL INFORMATION"
    Furthermore, if you answered yes to any of the questions 13-19, you only need to provide the completed copy of the medical questionnaire completed by your doctor, not your "medical records." Do not send anything other than exactly what is requested as it has no bearing and only makes the process more difficult for the ladies working the packet.

    they clearly state to send a copy of the training certificate on the ccw application.

    There was some issue in the past with people giving some very "unofficial" looking certificates, whatever that means. Also, guys were sending in very poor copies that were basically unreadable.

    I give my students two original copies on certificate blanks with a gold embossed seal and imprint.

    A photocopy of my certificate is definitely acceptable, but it makes it easier for the applicant to just put one in their packet for them.

    Also if your sending in your fingerprints in ink (not having them scanned)get you a second copy made and turn in two cards, thats what I did and they weren't rejected. It just gives you that second chance if the first card is unacceptable.

    Its also a GREAT idea if for some reasson you decide NOT to use the original certificate and use a copy for it to be notarized as "TRUE COPY", as mentioned by Brannon there were some "IFY" looking Certs. sent in before so its a good idea to pay the extra $5 to get a notary seal/stamp on a photocopy, eventhough that doesnt guarantee that they will be able to read it if the copy is JUNK.
