Suppressed would seem to be the first choice. Or of course some sort of electronic hearing protection might even be better (let the bad guys have the hearing damage if you miss!).

Considering this is an everyday problem for you, some kind of hearing aid with protection might be spending the money on ($600+).

I had a .357 mag fired from the passenger of my truck out the driver side window. I was the driver, and it was basically fired in my face... powder marks all on my headliner still... I was ringing for a good 6 hrs afterward. It had to be done, and I'm glad it was just my ears, and not my life. Ear doctor told me I have mild noise induced hearing loss from it, but I feel like I can still hear just fine. If it has to be done, you will be fine in the long run.

BTW, the guy that actually fired the guns ears were not hurting about 2 hrs before mine, so as the shooter, I think you would be fine.

I personally prefer quick drawing my Barrett .50 in intervehicular self defense situations. If you ears aren't bleeding you're doing it wrong!

I think you should look into an integrally suppressed pistol.

One problem there. Suppressor makes gun long. Not ideal for a carry gun.
